Planning appointments, rehabilitation and treatment journeys
Regular appointments, physiotherapy and rehabilitation often involve recurring dates, similar routes and ongoing support needs. Keeping reusable journey information can make each arrangement clearer.
Treatment or appointment location, floor, reporting point and contact details.
Weekly or monthly dates, start times and expected finishing times.
Starting point, pick-up and drop-off locations, building access and wheelchair information.
Whether accompaniment, waiting support, medicine collection or family updates are needed.
What to do if an appointment changes, bad weather occurs or there is a delay.
Confirm each service separately
Even for repeated journeys, confirm the date, time, service user's condition, availability and quotation each time. KareLynk does not automatically create transport or escort jobs without confirmation.
Check each outward and return journey, even on the same route
These are fictional examples of how to organise information, not established services or appointments. A clinic appointment, outward transport, return transport and escort confirmation are separate matters. Confirmation for one does not confirm another date or the return journey.
First appointment: the clinic has confirmed the date, and the transport provider has confirmed the outward journey. The return is still pending. Family must continue arranging the return rather than mark the entire day as arranged.
Second appointment: the clinic changes the date. Record the new date, then contact the people responsible for the original outward journey, return and escort separately to check which arrangements need changing. Do not carry forward old confirmations until each party provides a new confirmation.
A reusable per-appointment journey record
Copy this format for each appointment and keep it separate from other dates. Record the latest update time when plans change. Charges, changes and cancellation arrangements are subject to the relevant provider's confirmation.
Appointment date and latest clinic confirmation:
Outward journey: time / pick-up and drop-off locations / provider / pending or confirmed / reference number (if any):
Return journey: time or provisional arrangement / pick-up and drop-off locations / provider / pending or confirmed / reference number (if any):
Escort: not needed / to be confirmed separately; confirmed scope and responsible person:
Changes this time: old details → new details; who has been notified / whose reply is pending:
Last update time and contact on the day:
This record is for your own organisation. It is not a booking application or confirmation, and this page does not collect or store its contents. Do not include identity-card numbers, diagnoses or medical records. Confirm the recipient and purpose before passing information on.
Choose suitable transport
If the service user cannot use ordinary public transport, compare the eligibility and limitations of Rehabus, Hospital Authority Non-emergency Ambulance Transfer Service, wheelchair taxis and private transport.
The Hong Kong Society for Rehabilitation's public FAQ explains bookings over multiple dates, reference numbers for individual journeys and how to change details. The template here is KareLynk's general organising suggestion, not a Rehabus form or booking system. Each provider's latest guidance governs eligibility, procedures and confirmation.
